Job summary

Elaya Health in New York is seeking a Nurse Practitioner or Physician Assistant to deliver on-site endocrine care for residents in skilled nursing and long-term care facilities across New York and New Jersey. You will manage diabetes, thyroid disease, and other endocrine conditions with strong clinical autonomy, flexible scheduling, and robust administrative support within a growing specialty practice led by Medical Director Dr.

Don Zwickler.

Qualifications

• Licensed NP or PA in NY and/or NJ; licensure required.
• Experience in endocrinology, internal medicine, geriatrics, or SNF preferred.
• Strong clinical judgment and communication skills.
• Ability to work independently within a larger clinical team.
• Patient-centered care for long-term care residents.

Responsibilities

• Perform on-site evaluations and follow-up visits.
• Manage diabetes, thyroid disorders, and endocrine conditions.
• Review glucose trends, labs, and medications.
• Collaborate with nursing staff, facility leadership, and PCPs.
• Document in facility EMRs with training and administrative support.
• Help reduce avoidable hospitalizations through proactive specialty care.

Salary: $150,000–$220,000 annually

Elaya Health is a rapidly growing endocrinology practice specializing in providing high-quality care to patients in skilled nursing and long-term care facilities. We are seeking an experienced and compassionate Nurse Practitioner or Physician Assistant to join our expanding team.

This is an excellent opportunity for a provider looking for a high level of clinical autonomy, flexible scheduling, strong administrative support, and the opportunity to practice within a growing specialty group.

About the Role

Our providers deliver on-site endocrine care to residents of skilled nursing and long-term care facilities throughout New York and New Jersey.

You will manage patients with diabetes, thyroid disease, and other common endocrine conditions while working closely with facility nursing teams, primary care providers, and administration.

Responsibilities

• Perform on-site patient evaluations and follow-up visits

• Manage diabetes, thyroid disorders, and other common endocrine conditions

• Review blood glucose trends, laboratory results, and medication regimens

• Optimize diabetes management and help reduce unnecessary finger sticks and medication burden

• Collaborate with nursing staff, DONs, facility leadership, and primary care teams

• Provide clear clinical recommendations and ongoing follow-up

• Document in facility EMRs, with training and administrative support provided

• Help reduce avoidable hospitalizations through proactive specialty care

Qualifications

• Licensed Nurse Practitioner or Physician Assistant in New York and/or New Jersey, or willingness to obtain licensure

• Experience in endocrinology, internal medicine, geriatrics, or skilled nursing preferred

• Endocrinology experience is a plus but not required for the right candidate

• Strong clinical judgment and communication skills

• Comfortable working independently while collaborating with a larger clinical team

• Patient-centered and committed to providing high-quality care to the long-term care population
