Qualifications

Responsibilities

• Perform on-site evaluations and follow-up visits at skilled nursing facilities.
• Manage diabetes, thyroid disorders, and other endocrine conditions.
• Review glucose trends, labs, and medication regimens.
• Optimize diabetes management and reduce finger sticks and burden.
• Collaborate with nursing staff, DONs, leadership, and PCPs.
• Provide clear clinical recommendations and follow-up.
• Document in facility EMRs with training support.
• Help reduce avoidable hospitalizations through proactive care.

Compensation: $150,000 – $220,000 annually

Elaya Health is a rapidly growing endocrinology practice specializing in high-quality care for patients in skilled nursing and long-term care facilities. We are seeking an experienced, compassionate Nurse Practitioner or Physician Assistant to join our expanding team.

This is an excellent opportunity for a provider seeking a high level of clinical autonomy, flexible scheduling, strong administrative support, and the chance to grow within an established and respected specialty group.

About the Role

Our providers deliver on-site endocrine care to residents of skilled nursing and long-term care facilities throughout the Cincinnati area. You will manage patients with diabetes, thyroid disease, and other common endocrine conditions while collaborating closely with facility nursing teams, primary care providers, and administration.

What You'll Do

• Perform on-site patient evaluations and follow-up visits
• Manage diabetes, thyroid disorders, and other common endocrine conditions
• Review blood glucose trends, laboratory results, and medication regimens
• Optimize diabetes management and help reduce unnecessary finger sticks and medication burden
• Collaborate with nursing staff, DONs, facility leadership, and primary care teams
• Provide clear clinical recommendations and ongoing follow-up
• Document in facility EMRs (training and administrative support provided)
• Help reduce avoidable hospitalizations through proactive specialty care

Qualifications

• Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in Ohio, or willingness to obtain licensure
• Experience in endocrinology (at least a year), internal medicine, geriatrics, or skilled nursing preferred
• Strong clinical judgment and communication skills
• Comfortable working independently while collaborating with a larger clinical team
