Job summary

Elaya Health in Columbus, OH is seeking a Nurse Practitioner or Physician Assistant to join our endocrinology and diabetes care team for on-site care in skilled nursing and long-term care facilities.

You will manage diabetes, thyroid disease, and other endocrine conditions, collaborate with nursing staff and primary care teams, and contribute to reducing hospitalizations through proactive specialty care.

Qualifications

• Licensed NP or PA in Ohio, or willingness to obtain licensure.
• Experience in endocrinology, geriatrics, or internal medicine preferred.
• Endocrinology experience is a plus, not required.
• Strong clinical judgment and communication skills.

Responsibilities

• Perform on-site patient evaluations and follow-up visits.
• Manage diabetes, thyroid disorders, and other endocrine conditions.
• Review blood glucose trends, labs, and medications.
• Optimize diabetes management and reduce finger sticks and burden.
• Collaborate with nursing staff, DONs, and primary care teams.
• Document in facility EMRs with training provided.
• Help reduce avoidable hospitalizations through proactive care.

Location: Columbus, OH area (on-site at skilled nursing and long-term care facilities)
Compensation: $150,000 – $220,000 annually

Elaya Health is a rapidly growing endocrinology practice specializing in high-quality care for patients in skilled nursing and long-term care facilities. We are seeking an experienced, compassionate Nurse Practitioner or Physician Assistant to join our expanding team.

This is an excellent opportunity for a provider seeking a high level of clinical autonomy, flexible scheduling, strong administrative support, and the chance to grow within an established and respected specialty group.

About the Role

Our providers deliver on-site endocrine care to residents of skilled nursing and long-term care facilities throughout the Columbus area. You will manage patients with diabetes, thyroid disease, and other common endocrine conditions while collaborating closely with facility nursing teams, primary care providers, and administration.

What You'll Do

• Perform on-site patient evaluations and follow-up visits
• Manage diabetes, thyroid disorders, and other common endocrine conditions
• Review blood glucose trends, laboratory results, and medication regimens
• Optimize diabetes management and help reduce unnecessary finger sticks and medication burden
• Collaborate with nursing staff, DONs, facility leadership, and primary care teams
• Provide clear clinical recommendations and ongoing follow-up
• Document in facility EMRs (training and administrative support provided)
• Help reduce avoidable hospitalizations through proactive specialty care
