Job summary

U.S. Department of Veterans Affairs in Augusta, GA seeks a full-time Social Worker within the Primary Care Mental Health Integration (PCMHI) program to serve as a Collaborative Care Manager (CoCM), supporting evidence-based, measurement-driven mental health services in primary care.

The incumbent works at the advanced practice level, coordinating care across primary care, mental health, and psychiatry teams with a focus on veterans.

Qualifications

• Master's degree in social work from a CSWE-accredited program.
• Must be licensed or certified to independently practice social work at the master's degree level.
• GS-9 requires one year post-MSW experience and licensure; GS-11 requires licensure and experience.

Responsibilities

• Conduct structured psychosocial assessments and develop treatment plans.
• Coordinate patient care and follow-up under supervision of licensed staff.
• Support engagement and care management through structured follow-up and data collection.
• Participate in interdisciplinary case conferences, team huddles, and care planning.
