Mindoula Health’s Collaborative Care Program (CCP) is expanding to Oregon. The Behavioral Health Integration Clinician– Associate Level is a core member of the CCP team, partnering with PCPs and psychiatric consultants to deliver short-term, evidence-based care.

Under supervision, you will screen and assess for mental health and substance use disorders, provide brief interventions, support medication adherence, educate patients, and document in the EHR while engaging clients across in-person,

Qualificações

• Master’s degree in Social Work, Counseling, Psychology, or related field.
• CSWA credential or licensure as applicable.
• 3+ years of experience with adult populations having chronic medical and behavioral needs.
• Experience with EMRs and telehealth workflows.
• Located in Oregon; majority of services are remote.

Responsabilidades

• Screen and assess for mental health and substance use disorders.
• Provide short-term, evidence-based interventions (BA, PST, MI).
• Support medication adherence and monitor side effects.
• Provide education and self-management support.
• Track clinical response via in-person, video, or phone and adjust plan as needed.
• Refer for specialty care when appropriate.
• Participate in weekly psychiatric case consultation.
• Document in the EHR according to workflow.
• Re-engage patients who fall out of care.
• Practice under supervision of independently licensed clinicians per Oregon regulations.
