Qualifications

• Bachelor’s degree required in Human Services or related field.
• Minimum of one year of post-graduate experience required.

Responsibilities

• Provide case management services to assigned consumers in accordance with individualized treatment and service plans.
• Coordinate and monitor services to support consumers’ behavioral health, medical, social, and community resource needs.
• Maintain accurate, timely, and current documentation in compliance with agency, state, and federal requirements.
• Collaborate with consumers, families, providers, and community partners to ensure continuity of care.
• Assist consumers with accessing community resources, benefits, and supportive services.
• Participate in treatment team meetings and contribute to care planning efforts.
• Adhere to all Pathways policies, procedures, and confidentiality standards.
• Perform additional duties as assigned within the scope of the position.
