Job summary

Mending Point Behavioral Health, a program under The Methodist Home of Kentucky, seeks a Targeted Case Manager to assess client needs, coordinate services, and support families in the community. The ideal candidate holds a Bachelor's degree in the human services field with at least 1 year of post‑bachelor behavioral health experience, plus the required DBH training.

Qualifications

• Bachelor’s degree in the human services field with at least 1 year of post-bachelor behavioral health experience.
• Must complete DBH/DD & IDD approved trainings per 908 KAR 2:260.
• Compliance with Medicaid standards in service planning and coordination.

Responsibilities

• Assess client and family needs and link to appropriate community resources.
• Develop, implement, and review individualized service plans.
• Coordinate with local resources to address client service needs.
• Manage an assigned caseload and ensure timely documentation and deadlines.
• Maintain compliance with Medicaid requirements.
