Pathways Inc. - Kentucky is hiring a Care Coordinator to ensure consumers receive integrated care via our Certified Community Behavioral Health Clinic. This full-time position requires a compassionate professional who can coordinate services and connect individuals to essential community resources for their wellness and recovery.
Candidates must have an Associate Degree or Diploma in Nursing and a current Kentucky nursing license. Experience in public health and care coordination is preferred. We encourage applications from those committed to person-centered care.

Salary: $44,000 annually
Pathways is seeking a compassionate, organized, and detail-oriented Care Coordinator to support individuals receiving services through our Certified Community Behavioral Health Clinic (CCBHC). The Care Coordinator plays a vital role in ensuring consumers receive seamless, integrated care by connecting them with behavioral health, physical health, and community-based resources that promote wellness, recovery, and overall quality of life. This position works collaboratively with consumers, providers, and community partners to coordinate services and support a person-centered plan of care.
This is a full time position
