Job summary

Adventist Health Tillamook is hiring a Physical Therapist to join our care team. The role focuses on evaluating patients, planning and delivering physical therapy, and coordinating discharge planning within interdisciplinary teams.

The position requires a DPT or foreign equivalent if licensed after 2016, state PT licensure, and CPR/BLS certification. Pediatric and geriatric populations may be served in a rural community setting.

Qualifications

• DPT degree required; only if license obtained after Jan 2016, or foreign equivalent.
• Two years of PT experience preferred.
• State licensure in Physical Therapy required.
• CPR or BLS certification required.

Responsibilities

• Completes evaluations and develops individualized treatment plans.
• Delivers therapy interventions and communicates discharge plans.
• Completes comprehensive clinical documentation within deadlines.
• Supervises assigned assistants.
• Performs other job-related duties as assigned.
