Job summary

Adventist Health Mendocino County is seeking a Physical Therapist to plan and conduct segments of treatment programs that restore and improve functional abilities. The role involves co-leading interdisciplinary patient groups and developing individualized treatment plans.

Required DPT degree (or foreign equivalent) and valid physical therapy licensure in applicable states, plus CPR/BLS certification and a valid driver's license.

Qualifications

• DPT degree required; only if obtained license on or after Jan 2016; or foreign equivalent.
• Licensure in Physical Therapy in all applicable states of practice required
• CPR or BLS certification from approved AH vendor required
• Valid driver's license and 21+ years old required
• Basic Life Support (BLS) or CPR certification from AH policy required
• Physical Therapy licensure in state of practice required

Responsibilities

• Performs evaluations to guide treatment plans and address areas needing attention.
• Provides therapy interventions using established protocols and structures individualized care and discharge plans.
• Completes comprehensive and accurate clinical documentation within deadlines.
• Supervises assigned assistants.
• Performs other job-related duties as assigned.
