Job summary

Adventist Health Tillamook is seeking a licensed Physical Therapist to plan and conduct segments of treatment programs designed to restore and improve physical, social and mental functions. You will serve as a co-leader in an interdisciplinary approach to patient groups, contributing to evaluation, treatment and discharge planning.

The role requires a Doctor of Physical Therapy (DPT) degree with licensure in the practice state; CPR/BLS certification is also required.

Qualifications

• DPT degree required; licensure in state of practice required or foreign equivalent.
• Two years' Physical Therapy experience preferred.
• CPR or Basic Life Support (BLS) certification required.
• Facility-specific BLS certification required per AH policy.

Responsibilities

• Completes evaluations to establish individualized treatment plans.
• Provides therapy interventions using established protocols and techniques.
• Establishes care and discharge plans and communicates them to relevant parties.
• Completes comprehensive clinical documentation within deadlines.
• Supervises assigned assistants and performs other duties as assigned.

Job Requirements

Doctor of Physical Therapy (DPT) degree: Required; only if obtained license on or after January 2016; or foreign equivalent.

Two years' Physical Therapy experience: Preferred.

Licenses/Certifications: Physical Therapy licensure in state of practice: Required Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy: Required.

Facility Specific License/Certifications: Basic Life Support (BLS) certification from approved vendor per AH policy: Required.

Essential Functions

Completes evaluations to ensure quality standards and addresses assessment areas that require attention. Uses appropriate clinical techniques and tools and establishes individualized treatment plans for patients. Provides therapy interventions using appropriate treatment protocols and techniques. Establishes individualized care and discharge plans and ensures plans are communicated to appropriate parties. Completes comprehensive and accurate clinical documentation within established deadlines. Supervises assigned assistants. Performs other job-related duties as assigned.
