You will assess needs, develop IRPs, and provide crisis intervention within a supportive, recovery-focused team. The role requires collaboration with CSS teams, therapy groups, and case management, ensuring culturally competent care and adherence to program documentation timelines.

Qualifikationen

• Bachelor’s degree in social work, human services, counseling, psychology or criminal justice is required or preferred.
• CADC certification is required or preferred for this role.
• Master’s degree is preferred and can enhance candidacy.

• Provide direct services to individuals to secure and maintain permanent housing.
• Develop integrated treatment goals and interventions with clients and the team.
• Coordinate and manage services with the treatment team and providers.
