Polaris Rehabilitation & Care Center in Cheyenne, Wyoming is seeking a dependable PRN CNA Transportation Aide to help residents travel safely to medical appointments and other approved destinations.

You will join a compassionate team, supporting transfers, mobility, and personal care within your CNA scope. This role offers flexible PRN scheduling with hours based on resident appointments and facility needs.

Qualifications

• Active Wyoming CNA certification in good standing.
• Valid driver's license with acceptable driving record.
• Safe transfer and mobility-assistance skills.
• Dependability, patience, and strong communication.

Responsibilities

• Safely drive residents to appointments and other approved destinations.
• Assist with transfers, wheelchair securement, mobility, and personal care within CNA scope.
• Coordinate appointment schedules and paperwork with Nursing.
• Monitor resident comfort and promptly report changes or concerns.
• Complete transportation logs, vehicle safety checks, and routine vehicle cleaning.
• Provide additional CNA support when transportation assignments permit.
