About the Role:

The CNA Sub-Acute plays a critical role in delivering compassionate and high-quality care to patients in sub-acute healthcare settings, such as rehabilitation centers and long-term care facilities. This position focuses on supporting patients who require specialized care following hospitalization, ensuring their comfort, safety, and progress toward recovery. The CNA will work closely with nursing staff and other healthcare professionals to implement individualized care plans and monitor patient conditions. By providing essential daily living assistance and observing changes in patient status, the CNA contributes significantly to the overall treatment outcomes and patient satisfaction. This role demands a strong commitment to patient dignity, effective communication, and adherence to healthcare regulations and protocols.

Minimum Qualifications:

• Current and valid Certified Nursing Assistant (CNA) certification in California.
• High school diploma or equivalent.
• Completion of a state-approved CNA training program.
• Basic knowledge of patient care techniques and safety protocols.
• Ability to pass a background check and meet health screening requirements.
• Minimum of 2 years of sub-acute care experience.

Preferred Qualifications:

• Experience working in sub-acute, rehabilitation, or long-term care settings.
• CPR and First Aid certification.
• Familiarity with electronic health record (EHR) systems.
• Strong interpersonal and communication skills.
• Additional training or certification in specialized care areas such as wound care or dementia care.

Responsibilities:

• Assist patients with activities of daily living including bathing, dressing, grooming, toileting, and feeding.
• Monitor and document patients’ vital signs, symptoms, and overall condition, reporting any changes to nursing staff promptly.
• Support mobility and rehabilitation exercises as directed by physical therapists or nursing staff to promote patient recovery.
• Maintain a clean, safe, and comfortable environment for patients, adhering to infection control and safety standards.
• Collaborate with interdisciplinary teams to ensure coordinated and comprehensive patient care.
• Provide emotional support and reassurance to patients and their families, fostering a positive care experience.
• Assist with the preparation and transfer of patients for medical procedures or therapy sessions.
