Job summary

Crossroads Hospice & Palliative Care is seeking a Quick Response CNA (STNA) to join our Even More Care program. You will respond quickly to urgent needs, provide comforting presence, and support families during end-of-life care.

Weekend shifts are available, and travel to Green, Ohio is required as we serve patients across 9 counties in Northeast Ohio. Qualifications include a HS diploma or GED, 75-hour OH competency, active CNA certification, and 1 year hospice or nurse aide experience.

Qualifications

• High School Diploma or GED required.
• Must complete Ohio Dept. of Health 75-hour competency test.
• Licensed CNA with unrestricted active certification.
• At least 1 year of Hospice or Nurse Aide experience required.
• Hospice or Home Health experience preferred.
• Strong interpersonal skills and ability to work with families and care team.
• Ability to learn clinical software and electronic records.
• Reliable vehicle, valid driver's license, and auto insurance.

Responsibilities

• Provide urgent support during active stages of dying as part of the care team.
• Be a steady presence for patients and families, addressing evolving needs.
• Travel to Green, OH office and serve patients across multiple counties.

Education

You will serve as the steady presence families can count on, responding quickly to evolving physical or emotional needs. CNA STNA Weekends Available Shifts:

• Saturday & Sunday, 8:00 AM to 8:30 PM
• Saturday to Sunday, 8:00 PM to 8:30 AM

Travel to the office located in Green, Ohio is required. Serving patients throughout 9 counties in Northeast Ohio as part of the emergent care team.

CNA STNA Weekends Qualifications:

• Must have a High School Diploma or G.E.D.
• Must have completed an Ohio State Department of Health Competency Test from an accredited program with a minimum requirement of 75 hours of certification
• Must be certified by the state as a nurses' aide with an unrestricted, active aide certification
• 1year of current experience as a Hospice Aide or Nurses Aide is required
• Hospice or Home Health Agency experience preferred
• Must possess interpersonal skills sufficient to interact effectively with patients and their families, as well as all interdisciplinary caregivers, peers, subordinates, and supervisors
• Must exhibit or demonstrate the ability to learn computer skills related to required clinical software and electronic clinical record utilization
• Must have reliable vehicle/transportation, valid driver's license, required automobile insurance, and the ability to safely and independently operate said vehicle
