Job summary

Crossroads Hospice & Palliative Care is seeking a Quick Response STNA/CNA for the Greater Cleveland area. You will be a steady presence, offering compassionate care to patients and support to families during end-of-life moments.

The role requires 3–6 months hospice or home health experience, Ohio CNA certification, and completion of the 75-hour Ohio competency program. A valid driver’s license and reliable vehicle are essential, with travel to Lorain and surrounding counties expected.

Qualifications

• Must have completed an Ohio State Department of Health Competency Test from an accredited program with a minimum of 75 hours.
• Must be certified by the state as a nurse's aide with an unrestricted, active aide certification.
• 3-6 months of current experience as a Hospice Aide or Nurses Aide required.
• Hospice or Home Health Agency experience preferred.
• Must possess interpersonal skills sufficient to interact effectively with patients, families, and care teams.
• Must be able to learn computer skills related to required clinical software and electronic records.
• Must have reliable vehicle/transportation, valid driver’s license, insurance, and the ability to operate said vehicle.

Responsibilities

• Provide urgent support during active stages of dying as a steady presence for patients and families.
• Serve the Greater Cleveland area, traveling to office in Valley View, and caring for patients across multiple counties.

As a Quick Response CNA (STNA) serving Greater Cleveland, you will be a vital part of our Even More Care program, providing urgent support during active stages of dying. You will serve as the steady presence families can count on, responding quickly to evolving physical or emotional needs. STNA/ CNA Qualifications:

• Must have reliable vehicle/transportation, valid driver's license, required automobile insurance, and the ability to safely and independently operate said vehicle.

STNA/ CNA Schedule & Benefits:

• Monday to Friday, 3:00 PM -11:30 PM
• Travel to the office in Valley View, Ohio is required. Serving patients throughout Lorain, Cuyahoga, Geauga, Lake, Ashtabula, and Trumbull Counties.
