Job summary

Holland Lake Rehabilitation & Wellness Center in Weatherford, TX, is seeking a Restorative Aide/CNA to provide routine restorative nursing care per the resident plan and supervisor direction.

Qualifications

• Certified Nursing Assistant in good standing in Texas.
• Completion of a restorative nurse aide training program preferred.
• Six months of experience in long-term care as a restorative aide preferred.

Responsibilities

• Provides residents with routine restorative nursing care and services in accordance with the resident’s assessment, care plan and as directed by supervisors.
• Documents care on Rehabilitation/Restorative flow sheet.
• Notifies restorative nurse coordinator or appropriate therapy department of any problems, referrals or reassessments needed.
• Reports significant change in resident’s condition or motivational level to the charge nurse.

RESTORATIVE AIDE/CNA
