OPS LIVING LLC in Bloomington, IN is seeking a Part-Time Certified Nursing Assistant to provide compassionate, hands-on resident care at Commons at Jackson Creek. You will follow the care plan and support residents with daily activities in a safe, homelike environment.

The role requires current CNA licensure, CPR/BCLS/First Aid, and strong communication. You will document care in the EHR, participate in care plan meetings, and work collaboratively with the Wellness team to ensure quality,

Qualifications

  • High School diploma or GED required.
  • Current CNA licensure.
  • CPR/BCLS/First Aid certification required.
  • Prefer experience with seniors.

Responsibilities

  • Provide hands-on care per the resident care plan.
  • Review and input into the resident care plan.
  • Document resident condition in the EHR and report to Wellness team.
  • Respond to security, call bells promptly, and take appropriate action.
  • Assist with toileting and incontinence care as needed.
  • Maintain HIPAA compliance and patient confidentiality.
  • Follow OSHA standards and infection control precautions.
  • Communicate effectively in English with residents and team.
  • Attend care plan and department meetings; organize tasks efficiently.

Position Summary: In compliance with regulation and company policy, CNA provides hands-on care, in alignment with the resident's care plan, in a safe homelike environment. The CNA is responsible for the direct, quality care for each resident.

Facilitate Enriching Community Environment:
  • Provides services to residents as directed by the care plan and/or Director of Health & Wellness
  • Reviews and provides input into the resident care plan
  • Reports on the condition of residents to the Wellness team as well as documents in EHR
  • Responds to security system quickly and appropriately and answers call bells immediately, taking appropriate action and resetting call bell
  • Assists with toileting and disposes of incontinence products properly to ensure sanitation
  • Fosters resident rights in each resident interaction
  • Assists residents with ADLs as identified in care plans
  • Implements OSHA standards and safety standards in assisting residents and completing daily work
  • Reports accidents immediately and follows a process for documentation and treatment
  • Understands and practices the proper method of care, disposal, and exposure risk to bloodborne pathogens, bodily fluids, infectious materials, sharp sticks, and hazardous materials
  • Implements universal precautions of cleanliness, hygiene, and health standards
  • Attends regular department/community meetings, cross over meetings and care plan meetings
  • Maintains strict confidentiality of client information in compliance with HIPAA requirements
  • Complies with Infection Control, Standard Precautions, and OSHA standards
  • Ability to work effectively within role independently and with other team members
  • Organize and complete work in a timely manner
  • Ability to read, write and effectively communicate in English

  • Wash resident laundry as required
  • Complete assigned cleaning tasks
Physical Activity & ADA Requirements
  • Standing for 4 to 6 hours at one time within 8 hour day
  • Sitting 1 to 2 hours at one time within 8 hour day
  • May support/assist up to 50 lbs frequently and 150 lbs occasionally
  • May lift/carry 40 lbs frequently and 70 lbs occasionally; the height of 3 to 4 ft for a distance of 30 yds
  • Repetitive use of hands for simple grasping, pushing, and pulling, fine manipulation
  • Bend: frequently

QualificationsExperience & Qualifications:
  • Enthusiasm for life and adding value to the lives of others
  • High School diploma/ GED required
  • Current CNA licensure
  • Prefer prior experience working with seniors
  • Current CPR, BCLS, and First Aid Certification
  • Strive for excellence. Previous experience focusing on customer service
