Responsibilities

• Monitor and document residents' vital signs and well-being
• Assist with mobility, transfers and positioning
• Support meal service and ensure nutritional needs are met
• Maintain a clean, safe and comfortable environment
• Communicate changes in residents' conditions to nursing staff
• Provide medication reminders as directed by providers
• Contribute to a positive, supportive atmosphere for residents and families
