Job summary

Chapters Health seeks a dedicated Certified Nursing Assistant to support home care patients in Florida. You will follow the primary nurse's instructions, perform personal care, and assist families with safe patient care while documenting observations and interventions for the care team.

Required credentials include a Florida CNA license, CPR, a valid Florida driver's license, and successful completion of a competency evaluation program. BLS and Alzheimer's/Dementias training are also required.

Qualifications

• High school diploma or GED is required.
• Certificate of completion of a qualified CNA training program.
• Current Florida CNA license is required.
• Must complete a competency evaluation program prior to furnishing services.
• CPR certification is required.
• Valid Florida Driver's License with good record.
• Automobile Insurance is required.
• Clear Level 2 background check.
• BLS certification for healthcare professionals (AHA/Red Cross).
• Alzheimer's/Dementias training completion through Florida DOEA.

Responsibilities

• Adhere to written and oral instructions prepared by the primary nurse.
• Provide all aspects of personal care and hygiene to patients per policy.
• Use safe techniques for ambulation, transfer, positioning, ROM, and standard precautions.
• Guide caregivers and families on safe personal care for patients (ADLs, mobility, environment).
• Document care, observations, and interventions to the primary nurse, managers, and IDG.

Home care visits for our participants to provide assistance with ADL's and IADL's

Role:

• Adhere to written and oral instructions prepared by primary nurse.
• Perform all aspects of personal care and hygiene to patients according to Hope Hospice Policy and Procedures.
• Utilize knowledge of proper and safe techniques, re: ambulation, transfer, positioning, and ROM, Standard precautions.
• Ability to guide caregiver and families regarding the safe and comfortable personal care of the patient inclusive of but not limited to ADL's, Standard Precautions mobility and environment. Assist families of patients as needed.
• Ability to document care and services, observations and interventions to primary nurse, managers and IDG.

Knowledge & Skills:

• Excellent communication; via phone, in person and email.
• Exhibit good time management skills and organizational skills.
• Attention to detail & ability to handle confidential information following HIPPA guidelines.
• Knowledge of meal preparation and feeding techniques and/or questioning.
• Must be flexible to work in other locations if needed.

Required qualifications:

• High school diploma or general education degree (GED)
• Certificate of completion of a qualified Certified Nurse Assistant training program.
• Current Florida Certified Nursing License
• Must also successfully complete a competency evaluation program prior to furnishing services to patients.
• CPR certification required.
• Valid Florida Drivers' License with good record
• Automobile Insurance
• Clear level 2 background check
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• Certification of completion of Alzheimer's Disease and Related Dementias Training through the Florida Department of Elder Affairs.

Compensation Pay Range:

$16.96 - $25.24
