Job summary

Chapters Health System is seeking a dedicated Certified Nurse Assistant to provide in-home personal care and support to clients in Florida. The role emphasizes compassionate, patient-centered care, adherence to nurse instructions, and safe mobility techniques.

You will document observations for the primary nurse, cooperate with families, and maintain HIPAA-compliant confidentiality. Eligible candidates must have a GED, CNA certification, valid Florida license, CPR, and a clean driving record.

Responsibilities

• Adhere to written and oral instructions prepared by primary nurse.
• Perform all aspects of personal care and hygiene to patients.
• Utilize knowledge of ambulation, transfer, positioning, and ROM safely.
• Guide caregivers and families in safe personal care, incl. ADLs and mobility.
• Assist families of patients as needed.
• Document care and interventions to the nurse, managers and IDG.

Role

Perform personal care and home making services for clients in the community.

• Adhere to written and oral instructions prepared by primary nurse.
• Perform all aspects of personal care and hygiene to patients according to Hope Hospice Policy and Procedures.
• Utilize knowledge of proper and safe techniques, re: ambulation, transfer, positioning, and ROM, Standard precautions.
• Ability to guide caregiver and families regarding the safe and comfortable personal care of the patient inclusive of but not limited to ADL’s, Standard Precautions mobility and environment.
• Assist families of patients as needed.
• Ability to document care and services, observations and interventions to primary nurse, managers and IDG.

Knowledge & Skills

Required qualifications

• High school diploma or general education degree (GED)
• Certificate of completion of a qualified Certified Nurse Assistant training program.
• Current Florida Certified Nursing License
• Must also successfully complete a competency evaluation program prior to furnishing services to patients.
• CPR certification required.
• Valid Florida Drivers’ License with good record
• Automobile Insurance
• Clear level 2 background check
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• Certification of completion of Alzheimer's Disease and Related Dementias Training through the Florida Department of Elder Affairs.

Compensation Pay Range: $16.96 - $25.24
