Job summary

Catholic Care Center is seeking a Certified Nurse Aide to join our care team. You will provide direct patient care under the supervision of an RN or LPN, perform ongoing data collection on residents, and assist with implementing individualized plans of care focusing on physical, psychosocial, and spiritual needs.

The position emphasizes safety, accurate documentation, and adherence to policies. CPR certification is preferred, and EEO practices apply to all associates and applicants.

Qualifications

• Data collection on all patients under RN/LPN direction.
• Direct patient care and reporting changes to RN/LPN.
• Identify safety hazards and initiate corrective action.

Responsibilities

• Performs initial and ongoing data collection on all patients/residents under the direction of the Registered Nurse/Licensed Practical Nurse.
• Implements plan of care developed by the RN based on patient needs.
• Provides direct patient care and reports condition to RN or LPN.
• Ensures resident safety by identifying hazards and emergencies and taking action.
• Documents care results per policies and procedures.
• Complies with ministry policies and procedures.
• Other duties as assigned.
