Job summary

Chapters Health System is seeking a home care aide to provide personal care and home-making services in patients’ homes throughout Florida. You will follow directions from the primary nurse, perform ADLs, mobility support, and document observations for the care team.

The role requires a CNA certificate, Florida CNA license, CPR certification, and a valid Florida driver’s license with a clean record. Background check will be conducted.

Qualifications

• High school diploma or GED.
• CNA certificate from a qualified training program.
• Current Florida CNA license.
• CPR certification is required.
• Valid Florida driver’s license with a good record.

Responsibilities

• Perform personal care and home making services for clients in the community.
• Adhere to written and oral instructions prepared by the primary nurse.
• Perform all aspects of personal care and hygiene per policy and procedures.
• Utilize knowledge of ambulation, transfer, positioning, ROM and standard precautions.
• Guide caregivers and families on safe, comfortable personal care incl. ADLs and mobility.
• Document care and services to the primary nurse, managers, and IDG.
