Job summary

Chapters Health System in Florida is seeking a Certified Nursing Assistant to support patient care in our hospice and home-care programs. You will follow instructions, perform personal care tasks, assist with mobility and ROM, and document observations for the primary nurse and care team.

Responsibilities include guiding families, maintaining safety protocols, and coordinating with the nursing staff. A valid Florida CNA license, CPR/BLS certification, and clear background check are required;

Qualifications

• Excellent communication across phone, in person, and email.
• Strong time management and organizational skills.
• Attention to detail and handling confidential information following guidelines.
• Knowledge of meal preparation/feeding techniques and ability to adapt to patient needs.

Responsibilities

• Adhere to written and oral instructions prepared by primary nurse.
• Perform all aspects of personal care and hygiene for patients per policy.
• Utilize proper techniques for ambulation, transfer, positioning, and ROM; observe standard precautions.
• Guide caregivers and families in safe, comfortable personal care and document observations and interventions.
