Responsibilities

• Provides direct patient care according to individualized care plans.

• Assists in transporting and transferring patients.
• Monitors and records vital signs, intake/output, and reports abnormalities.
• Performs hourly rounds and responds to call lights promptly.
• Maintains accurate and timely documentation.
• Supports hygiene and comfort, including turning/repositioning to prevent ulcers.
• Assists with feeding and hydration while following dietary restrictions.
• Collaborates with healthcare team to ensure continuity of care.
