Qualifications

• Current certification as a CNA in DE
• BLS certification
• High school diploma preferred or CNA license while a student

Responsibilities

• Provides direct care to patients per individualized care plans
• Assists with activities of daily living (bathing, oral care, dressing, grooming, toileting)
• Assist in transportation and transfer of patients for comfort and safety
• Monitor and record vital signs and inputs/outputs, report abnormalities
• Perform accuchecks as directed
• Document accurately and promptly
• Assist with patient hygiene and comfort; reposition to prevent ulcers
• Assist with feeding and hydration per dietary restrictions
• Collaborate with healthcare team for continuity of care
• Complete hourly rounds and respond to call lights
• Provide emotional support to patients and families
• Maintain clean patient environment; prepare rooms for admissions/transfers
• Adhere to infection control protocols
• Participate in ongoing education and training
