Qualifications

Responsibilities

• Provides direct care to patients in accordance with care plans.
• Assists with activities of daily living: bathing, dressing, grooming, toileting.
• Assists with transportation and transfers for patient safety and comfort.
• Monitors vital signs and reports abnormalities to nursing staff.
• Completes accuchecks and documents promptly and accurately.
• Maintains patient hygiene and comfort, including turning to prevent ulcers.
• Assists with feeding and hydration per dietary restrictions.
• Collaborates with healthcare team for continuous care and outcomes.
• Completes hourly rounds and responds to call lights and patient requests.
• Provides emotional support to patients and families.
• Keeps patient rooms clean and stocked, prepares rooms for admissions.
