Job summary

Princeton Rehabilitation and Health Care Center in Chicago seeks an Activities Assistant to implement the daily activity calendar and support the Activity Director in delivering a resident-centered program.

You will engage residents in social and recreational activities, document participation, assist with outings, and help maintain calendars and boards while following facility policies and state regulations.

Qualifications

• High school education or equivalent.
• At least 1 year in healthcare, volunteering or experience with disabled population.
• Knowledge of Federal and State activity regulations.
• Ability to read and write English.
• Bi-lingual abilities preferred.
• Must have a valid IL driver’s license.
• Empathy, compassion, and strong communication with residents, families, and staff.
• Ability to engage with ill, disabled, elderly, emotionally upset, or hostile individuals.
• Able to communicate with the community and health professionals.

Responsibilities

• Provide feedback to Activity Director for monitoring goals through a team approach.
• Lead a therapeutic, person-centered activity program aligned with the Activity Mission and Philosophy.
• Follow all Activity Policies and Federal/State regulations.
• Know each resident’s background to create person-centered activity plans.
• Plan daily activities reflecting residents’ schedules, choices and rights.
• Support language services and communication needs of residents.
• Conduct and document assessments (social, medical, cognitive, spiritual, etc.).
• Maintain records such as daily participation and progress notes.
• Update activity boards with monthly themes.
• Facilitate community involvement and plan resident outings with transportation as needed.
• Follow staff schedules and work collaboratively with team.
• Maintain structure and timeliness of activities; obtain permission before cancellations.
• Keep equipment and supplies well organized and report needs promptly.
• Update calendars and boards daily.
• Perform other duties as assigned.
