Frontier Hospice is seeking a Per Diem Hospice Aide (CNA) to provide hands-on end-of-life care in the Edmond area. The CNA will work closely with the nursing team to meet physical, emotional, and personal needs with dignity and empathy.

Qualified applicants will hold a CNA certificate, have at least one year of CNA experience (hospice a plus), and maintain CPR if required. A valid driver's license and reliable transportation are needed.

Qualificações

• High School Diploma or G.E.D. required.
• Holds current unencumbered CNA certificate in the state.
• Minimum one year of CNA experience in acute or long-term care; hospice experience a plus.
• CPR certification if state-mandated.
• Current driver’s license and access to dependable transportation.

Responsabilidades

• Provide personalized care following the plan of care and respond promptly to patient and family needs.
• Assist with personal care including bathing, oral hygiene, bedding, clothing, and shampooing.
• Maintain exceptional customer service and patient satisfaction at all times.
• Support families by sharing knowledge and guidance for at-home care of terminally ill loved ones.
• Maintain accurate documentation for patient care in the home and agency systems.

Responsibilities

• Provide Personalized Care: Follow the carefully crafted plan of care to ensure the comfort and well-being of our patients. Attend promptly to the requests of patients and their family/caregiver, offering compassionate and unwavering support.
• Assist with Personal Care: Support patients with various personal care needs, including bathing, oral hygiene, changing bedding, clothing, and shampooing. Your presence during these intimate moments contributes significantly to their overall comfort and dignity.
• Be Dedicated to Customer Service: Emphasize exceptional customer service, going above and beyond to ensure optimal patient satisfaction. Your dedication to providing outstanding care will be valued and celebrated as part of our team.

Qualifications

• High School Diploma or G.E.D. required.
• Holds current unencumbered certificate in the state working as a C.N.A.
• Minimum one (1) year experience as a C.N.A. in an acute care or long term care setting. Hospice exp a plus!
• Possess and maintains current CPR certification if state-mandated
• Current driver's license, vehicle insurance, and access to dependable transportation.

$17.50 - $20.00 per hour (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
