Tapestry Hospice & Palliative Care in North Georgia is seeking a compassionate Hospice Aide/CNA to join a dedicated care team. You will follow personalized care plans, be the heart and eyes of the patient, and coordinate with the RN Case Manager to exceed expectations.

The role requires a Georgia CNA license, CPR/First Aid certification, 1–2 years hospice or related experience, a valid driver's license and reliable vehicle, and excellent time management. We offer competitive pay and benefits.

Description:

As a Hospice Aide/Certified Nursing Assistant (CNA), you are one of the most vital components of the Interdisciplinary Team! You are the heart, the eyes, and the key communicator of your patient’s needs. Your role is to follow a carefully planned out, personalized Care Plan for each patient you are assigned and to r eport and work closely with the RN Case Manager to ensure the care of the patient is completed and expectations exceeded. You will effectively interact with all members of the Interdisciplinary Team as well as with your patients and their loved ones while maintaining the highest standards of care and professional nursing clinical competency.

Minimum Requirements:

• Current CNA License in the State of Georgia
• Current CPR & First Aid Certification
• 1-2 years hospice experience (other candidates may be considered with experience in assisted living, home health, long term care)
• Utmost compassion & integrity
• Driver’s license, insurance and vehicle
• Excellent Time Management Skills
