Job summary

Alumus in Mesa, AZ seeks a Certified Nursing Assistant to support patients at home under RN supervision. You will perform personal care tasks and assist with admissions, transfers, and discharges in line with care plans to ensure comfort and safety.

Requirements include a CNA license, current CPR and fingerprint clearance, with a minimum of six months CNA/LNA experience; LTC/SNF or Rehab exposure is preferred.

Qualifications

• CNA license certification prior to hiring.
• Must have a current CPR and fingerprint clearance card.
• Minimum of six (6) months CNA/LNA experience preferred.
• Recent Long-Term Care/ Skilled Nursing Facility or Transitional Rehab experience) LTC/SNF/Rehab preferred.

Responsibilities

• Assist guests/patients during admissions, transfers, and discharges; check each guest/patient to ensure personal care needs align with wishes and care plan.
• Review care plans daily to determine changes in the patient’s daily care routine.
