Job summary

Alumus in Chandler, AZ is seeking a Certified Nursing Assistant to assist guests during admissions, transfers and discharges, and to follow care plans with nursing supervision. The role emphasizes patient advocacy and holistic care in residential settings.

Qualifications

• CNA license certification prior to hiring.
• Must have a current CPR and fingerprint clearance card.
• Minimum of six (6) months CNA/LNA experience preferred.
• Recent Long-Term Care/ Skilled Nursing Facility or Transitional Rehab experience
• LTC/SNF/Rehab preferred.

Responsibilities

• Assist guests/patients during admissions, transfers, and discharges. Check each guest/patients routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes and the care plan.
• Review care plans daily to determine if changes in the guests/patient's daily care routine have been made on the care plan.
