About the Role

As a Private Duty Home Health Aide, you will provide one-on-one care to patients in the comfort of their own homes, supporting their health and well-being under the supervision of a Registered Nurse or Therapist, following physicians' orders. Our service areas include Milwaukee, Waukesha, Ozaukee, and Washington Counties, with mileage reimbursed between visits.

Responsibilities

1. Review care plans prior to providing care, and notify the Case Manager of any issues or needs for revision.
2. Assist with daily activities such as bathing, dressing, transfers, range of motion exercises, and toileting.
3. Explain procedures to patients and involve them in their care.
4. Complete mobile care tasks upon arrival and departure, accurately logging mileage and tasks as per agency policy.
5. Collaborate effectively with patients, families, care providers, and agency staff, communicating patient needs and reporting pertinent information promptly.
6. Perform home maintenance tasks like cleaning, laundry, meal prep, and transportation as outlined in the care plan.

Qualifications

Details

Full-time, 40 hours/week, day shift from 8:00 am to 8:00 pm, including weekends and holidays. Hourly, non-exempt position.
