Position: Private Duty Home Health Aide

As a Home Health Aide, you will assist patients with personal care, support their health maintenance, facilitate treatment under physician orders, and contribute to a positive health environment. All activities are performed under the supervision of a Registered Nurse or Therapist.

1. Review care plans prior to providing care; notify the Case Manager of any issues or needed revisions.
2. Assist with daily living activities such as bathing, dressing, transfers, range of motion exercises, and toileting, under the supervision of the Case Manager.
3. Explain care procedures to patients and involve them in their care.
4. Complete mobile care tasks upon arrival and departure from patients' homes, including accurate mileage logs.
5. Collaborate with patients, family members, and care team members, reporting pertinent information promptly to the Case Manager.
6. Perform home maintenance tasks as outlined in the care plan, including cleaning, laundry, meal prep, and transportation.

Qualifications

• Wisconsin CNA certification for work in federally certified agencies
• Prior experience working with the public
• Valid Wisconsin Driver's License and reliable, insured vehicle

Details

Full-time (40 hours/week), day shift from 8:00 am to 8:00 pm, including weekends and holidays. Hourly, non-exempt position. Private duty role.
