Position: Private Duty Home Health Aide

As a Home Health Aide, you will assist patients with personal care, support their health maintenance, facilitate treatments under supervision, and foster a healthy environment, all in accordance with physicians' orders and the guidance of a Registered Nurse or Therapist.

1. Review the care plan prior to providing care, and communicate any concerns or needs for revisions to the Case Manager.
2. Assist with daily activities such as bathing, dressing, transfers, range of motion exercises, and toileting under the supervision of the Case Manager.
3. Explain care procedures to patients and involve them in their care.
4. Complete mobile care tasks and mileage logs accurately and promptly upon arrival and departure.
5. Collaborate with patients, family members, care providers, and agency staff, ensuring timely communication of patient needs and pertinent information.
6. Perform home maintenance tasks as outlined in the care plan, including cleaning, laundry, meal prep, and transportation.

Minimum Requirements:

• Wisconsin CNA certification for work in Federally Certified Home Health Agencies and Hospices
• Prior experience working with the public
• Current/valid Wisconsin Driver's License, reliable and insured vehicle

Additional Details:

Full-time, 40 hours/week, Day Shift from 8:00 am to 8:00 pm, including weekends and holidays. Hourly, non-exempt position. This is a Private Duty role.
