Southeast in Red Wing, MN is seeking a CNA Home Health Aide to provide for the personal care needs of clients per the home health aide plan of care. You will report changes in condition, document observations, and complete required paperwork in a timely manner.

Responsibilities include following care policies, ensuring client safety, and performing light household duties up to 20% of the time. Must be a Minnesota Registry CNA to qualify.

Qualifications

• Must be a CNA on the Minnesota Registry.
• CNA certification required for employment in Minnesota.
• Experience in home health care is a plus.

Responsabilités

• Provides for the personal care needs of the client as designated on the home health aide plan of care.
• Reports changes in the client’s condition and needs in a timely fashion.
• Documents cares given and pertinent observations accurately as directed by the Care Plan/RN Clinical Manager.
• Submits to the organization – completed documentation within established time parameters.
• Performs all duties within the boundaries of the personal care policies and procedures.
• Provides care to the client with attention to the safety needs of the client.
• Performs household duties as designated by the care plan, not to exceed more than 20% of the time.
