Job summary

Granite Peak Home Health in Montana seeks a compassionate Home Health Aide to support patients in their homes under RN supervision. You will perform personal care, assist with daily living activities, and help monitor patients' condition.

The role requires completion of the Home Health Aide training, CPR certification, and adherence to Medicare conditions. Training is provided for CNAs; the team emphasizes respectful, patient-centered care and timely documentation of visits.

Qualifications

• Must meet Medicare conditions of participation for Home Health Aide training.
• If licensed CNA, Granite Peak Home Health provides Home Health Certification.
• Sympathetic attitude toward the care of the sick and elderly.
• Ability to read, write, and follow directions.
• Maturity to handle the demands of the job.
• Valid CPR certification required.

Responsibilities

• Understands and adheres to established Agency policies and procedures.
• Performs personal care, bath, and hands-on care as assigned.
• Completes appropriate visit records in a timely manner as per Agency policy.
• Reports changes in the patient's condition and needs to the RN.
• Performs household services essential to health care in the home as assigned.
• Ambulates and exercises the patient as assigned.
• Performs simple procedures as an extension of the therapy or nursing services, e.g., ROM exercises as assigned.
• Assists with medications that are ordinarily self-administered as assigned.
• Attends in-service and continuing education programs as scheduled and necessary.
• Attends patient care conferences as scheduled.

1. Must meet Medicare conditions of participation in the Home Health Aide training program and be competent. If currently a licensed CNA, GPHH can provide Home Health Certification
2. Have a sympathetic attitude toward the care of the sick and elderly.
3. Ability to carry out directions, read and write.
4. Maturity and ability to deal effectively with the demands of the job.
5. Must have valid CPR certification

Principal Responsibilities:

This position reports to Home Health Director and is supervised by RN’s, PT’s, OT’s, SLP’s

1. Understands and adheres to established Agency policies and procedures.
