Blize Healthcare is seeking a Certified Nursing Assistant or Home Health Aide to join our team. The role involves providing routine healthcare services to patients either in their homes or in care facilities within San Francisco County.

Responsibilities:

1. Assist patients with personal care such as bathing, dressing, and grooming.
2. Educate patients on using aids like canes, walkers, and special utensils for eating, as well as techniques and equipment for personal hygiene.
3. Record patient information and notify supervisors of any changes or unusual conditions.
4. Support patients in maintaining their independence at home by monitoring their condition, providing personal services, and educating family members.
5. Administer medications and change bandages as needed.
