Job summary

Transitions Healthcare Washington PA is seeking compassionate CNAs to join our caring team for full-time roles. A $3,000 sign-on bonus is offered through the end of the month. Candidates licensed in PA, OH, or WV are encouraged to apply, with eligibility to work in PA.

We provide a supportive environment focused on resident care and professional growth. We are not accepting relocations or temporary agency contracts at this time.

Qualifications

• Current Pennsylvania CNA certification required, or eligibility to work in PA with OH/WV credential.
• CPR certification preferred.
• Experience in long-term care, skilled nursing, rehab, or hospital setting is preferred but not required.

Responsibilities

• Provide resident-centered care including ADLs such as bathing, dressing, grooming, toileting, feeding, hygiene, and mobility.
• Assist with repositioning, transfers, and ambulation while maintaining comfort and dignity.
• Document vital signs, weights, intake/output, and other observations.
• Collect routine specimens as directed.
• Perform restorative nursing activities per care plans.
• Note changes in resident condition and report to licensed staff.
