Job summary

Transitions LifeCare in Raleigh, NC is hiring immediately for a Certified Nursing Assistant to provide compassionate, patient-centered care in home health and hospice settings.

You will follow individualized care plans, work with the Hospice Home team to meet goals, maintain patient comfort, privacy and dignity, and report important information to leadership. The role requires at least 1 year of experience and an active CNA listing. Reliable transportation is required.

Qualifications

• Current CNA listing with NC Home Health Aide Registry.
• Reliable transportation.
• Minimum of 1 year experience.
• Must submit and pass all applicable pre-employment and annual tests and meet all training and other applicable state and federal guidelines.

Responsibilities

Transitions LifeCare is hiring immediately. As one of the community’s industry leaders, we provide exceptional home health, hospice, and palliative care services throughout our 8-county territory. Transitions LifeCare’s goal is to care for individuals and families with compassion, support, and expertise as they navigate the end of life or life changing illness.

The schedule for this position is 3 shifts per month—either 7:00 am–7:30 pm or 7:00 pm–7:30 am.
