Responsibilities

• Deliver direct patient care, including bathing, dressing, grooming, toileting, and feeding.
• Assist with patient transfers, ambulation, and repositioning.
• Monitor and report changes in patient condition to the Charge Nurse.
• Document patient care and daily activities in line with regulations.
• Support care plans to improve patient outcomes, maintain safety and infection control.
• Participate in shift handoffs, training, and follow facility policies.
