Job summary

Lightways Hospice in Joliet, IL is seeking a Field CNA to provide in-home patient care under RN supervision. The role operates Monday–Friday, 8:00 AM to 4:30 PM, traveling to patients in Joliet and surrounding areas.

You will support personal care, mobility, and comfort, ensuring safety and documenting observations. The ideal candidate holds a high school diploma or GED, an Illinois CNA certification, and a valid driver's license with an insured vehicle.

Qualifications

About Lightways Hospice

Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness Care is an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois. We provide exceptional serious illness care, hospice care and grief support for adults and children.

The majority of our direct patient care is in the field with Lightways team members providing care in the home, nursing facility or assisted living facility. We also have a 24-bed inpatient unit in Joliet. Non-clinical staff are based out of our Joliet administrative offices.

Field CNA

The field CNA is a paraprofessional member of the interdisciplinary group who works under the supervision of a registered nurse and performs various services for a patient as necessary to meet the patient's personal needs and to promote comfort. The field CNA is responsible for observing the patient, reporting these observations and documenting observations and care performed. This role is Monday- Friday 8AM to 4:30 PM.

Key Responsibilities:

• Provide compassionate care, emotional support, and companionship to patients and families
• Assist with personal care, mobility, and daily living activities
• Monitor, document, and report patient condition and changes to the RN
• Ensure patient safety and proper use of equipment
• Maintain a clean, safe, and comfortable home environment
• Follow care plans, policies, and participate in training and team activities
  
• Position requires outside travel in Joliet and surrounding area

Education/Experience:

• High school diploma or GED.
• Meets the training requirements of Illinois.
• Current CNA certification.
• Requires a valid driver’s license and a dependable, properly insured vehicle that complies with state and organizational standards.
