Job summary

Lightways Hospice and Serious Illness Care is seeking a Field CNA to provide compassionate in-home patient care and support to families in Joliet and surrounding areas.

You will assist with personal care, mobility, and daily activities, while documenting changes for the RN and ensuring safety and comfort in patients' homes.

Qualifications

• High school diploma or GED.
• Meets the training requirements of Illinois.
• Current CNA certification.
• Requires a valid driver’s license and a dependable, properly insured vehicle that complies with state and organizational standards.

Responsibilities

• Provide compassionate care, emotional support, and companionship to patients and families
• Assist with personal care, mobility, and daily living activities
• Monitor, document, and report patient condition and changes to the RN
• Ensure patient safety and proper use of equipment
• Maintain a clean, safe, and comfortable home environment
• Follow care plans, policies, and participate in training and team activities
• Position requires outside travel in Joliet and surrounding area

The majority of our direct patient care is in the field with Lightways team members providing care in the home, nursing facility or assisted living facility. We also have a 24-bed inpatient unit in Joliet. Non-clinical staff are based out of our Joliet administrative offices.
