Job summary

Joliet Area Community Hospice is seeking a compassionate Certified Nursing Assistant to join the Lightways Inpatient Team in Joliet, IL. The role provides direct patient care on 12-hour shifts, with weekend and holiday rotations, within a supportive hospice setting.

You will assist with ADLs, monitor changes, document care, and support families. Illinois CNA certification and a high school diploma or GED are required, with some long-term care experience preferred.

Qualifications

• CNA certification required with IL practice.
• Experience in hospital, nursing facility, or long-term care preferred.
• High school graduate or GED with willingness to complete required training.

Responsibilities

• Provide compassionate care, emotional support, and companionship to patients and families
• Assist with personal care, mobility, and activities of daily living

Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness Care is an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois. We provide exceptional serious illness care, hospice care and grief support for adults and children.

The majority of our direct patient care is in the field with Lightways team members providing care in the home, nursing facility or assisted living facility. We also have a 24-bed inpatient unit in Joliet. Non-clinical staff are based out of our Joliet administrative offices.

CNA - Inpatient

As a member of this dynamic team, your responsibilities would include providing quality patient care to our patients and assisting them with their ADL’s. Prior experience in hospital setting, nursing home or long-term care preferred.

Our Inpatient Unit team works 12-hour shifts in our facility. The position works three (3) shifts a week from 7am – 7:30pm with rotating weekends and holidays.

Key Responsibilities:

• Provide compassionate care, emotional support, and companionship to patients and families
• Assist with personal care, mobility, and daily living activities
• Monitor, document, and report patient condition and changes to the RN
• Ensure patient safety and proper use of equipment
• Maintain a clean, safe, and comfortable home environment

Education/Experience:
