You will support ADLs, monitor vital signs, assist with mobility, and maintain clear documentation while collaborating with the healthcare team to ensure seamless care transitions and resident well-being.

Qualifications

• Must hold current Missouri CNA certification.
• High school diploma or equivalent required.
• Prior long-term care experience preferred.

Responsibilities

• Provide personal care including bathing, dressing, grooming, and feeding.
• Assist with vital signs, hygiene, and mobility as needed.
• Monitor changes in resident condition and report to nursing staff.
• Maintain accurate records and document treatments and medications.
• Collaborate with the healthcare team to develop individualized care plans.
