Job summary

Trinity Health PACE in Mishawaka, IN is seeking a CNA – Certified Nursing Assistant for the First Shift at the St Joseph PACE facility. The role offers a $2500 sign-on bonus with a 2-year commitment and benefits on day one.

You will provide personal care, assist with activities of daily living, support mobility, and help manage daily routines for elderly participants within a coordinated care model. A valid CNA license is required and travel may be involved.

Qualifications

• Must be an active certified nurse assistant in the state where PACE Organization is located.
• Minimum of one (1) year of documented experience with frail or elderly population preferred.
• Valid driver's license.
• Superior written/verbal communication skills.
• Customer focused, able to adapt to ongoing change.
• Proficiency with MS Office and basic email/calendar skills.
• May require 10% local travel and ability to lift 20 pounds.

Responsibilities

• Provide personal care assistance maintaining independence for daily living activities.
• Assist with mobility, exercise, and walking for participants.
• Provide behavioral supervision or support and follow nursing and medication instructions.
• Engage participants in conversations, recreation, meals, and light housekeeping.
