Job summary

Trinity Health in Mishawaka, IN seeks a part-time CNA to provide personal care and support for elderly participants at the St Joseph PACE facility. You will assist with activities of daily living, mobility, bathing, grooming and meal service, while promoting independence and safety in a compassionate, patient-centered environment.

Must hold an active Indiana CNA license, have at least one year of frail-elderly experience, a valid driver’s license, and strong written and verbal communication.

Qualifications

• Active CNA license in Indiana
• Minimum 1 year of frail/elderly care experience preferred
• Valid driver’s license
• Excellent written and verbal communication skills
• Ability to adapt to ongoing change and work independently
• Proficiency with Microsoft Office and email/calendar systems

Responsibilities

• Provide personal care assisting with activities of daily living (toileting, showers, eating, grooming)
• Assist with mobility, physical activity, and exercise
• Provide behavioral supervision or support (orientation, reminders, diet restrictions)
• Assist with conversations, recreation, meals, snacks, and housekeeping
