Qualifications

• Ability to lift, transfer, and reposition patients.

Responsibilities

• Provide direct patient care including vital signs, bathing, dressing and grooming.
• Assist with activities of daily living (toileting, feeding, ambulation).
• Maintain accurate and timely documentation of patient records.
• Collaborate with the care team to implement individualized care plans.
• Communicate effectively with patients, families, and healthcare professionals.
