Momentous Health at Vandalia is seeking a State Tested Nursing Assistant (STNA) for the Night Shift to provide compassionate direct patient care, monitor vital signs, assist with ADLs, and support the nursing team.

Qualifications

• Current STNA certification (or willingness to obtain)
• High school diploma or equivalent required

Responsibilities

• Provide direct patient care, including vital sign checks, wound care, and assistance with activities of daily living (ADLs)
• Develop and implement individualized care plans, working closely with our interdisciplinary team to ensure seamless care delivery
• Maintain accurate and detailed records of patient care, medication administration, and treatments
• Assist with transportation, discharging patients, and supporting nursing staff with daily tasks
• Participate in quality improvement initiatives, identifying and addressing areas for improvement
• Collaborate with other healthcare professionals to ensure effective communication and coordinated care
