Job summary

Trilogy Health Services, LLC is seeking a Certified Nursing Assistant (CNA) to provide daily nursing care and companionship to residents according to each resident’s plan of care. CNAs will work with fellow caregivers to deliver respectful, personalized support and ensure comfort and safety on site.

Responsibilities include assisting with activities of daily living, observing resident conditions, and collaborating with the care team to promote wellbeing.

Qualifications

• High School Diploma or GED/HSE preferred.
• Active state CNA Certification.
• 0-1 years of relevant experience preferred.

Responsibilities

• Help residents with daily living activities (ADLs) such as hygiene, turning, lifting, and transporting.
• Observe and report changes in residents' condition and make independent decisions when needed.
• Collaborate with the team to ensure resident comfort and well‑being.
• Provide companionship, compassion and encouragement to residents.
• Maintain a safe and comfortable environment.
